Case Management Coordinator Opportunity — Community Health Worker (WellSpan)
ASC People is recruiting for a Case Management Coordinator to serve as a Community Health Worker supporting WellSpan. This role focuses on outreach, care coordination, and connecting members to preventive health services and community resources across Adams, Lancaster, Lebanon, and York Counties, Pennsylvania.
The successful candidate will build relationships with members, providers, and community organizations, conduct outreach related to preventive health and care gaps, and help address the social and health-related barriers that impact member wellbeing.
Key Responsibilities
- Initiate outreach efforts, both face-to-face and telephonically, related to targeted preventive health, care gaps, maternity, care transitions, and chronic condition initiatives.
- Educate members on preventive health services, assess opportunities for care management intervention, and make appropriate referrals to community resources.
- Develop rapport with members and community-based organizations, collaborating with groups such as Head Start Programs, WIC, community centers, and homeless shelters.
- Contribute to educational materials for members and providers, and report on outreach outcomes to management and peers.
- Participate in developing effective, measurable, and innovative programs targeting at-risk members, including conducting SDOH assessments and assisting with scheduling and social health barriers.
- Maintain understanding of the organization’s lines of business and member benefits to provide accurate, current information.
- Represent the company at community events, including those sponsored by community partners.
Qualifications
- High School Diploma or GED required.
- 1 year of Community Health Worker experience or related field.
- Lived experience in the community served.
- Certified Community Health Worker (CCHW) – PA Certification Board, required within 12 months of start date.
- Bilingual — Spanish & English required.
Preferred Qualifications
- Experience in Medicaid regulations.
- Experience in customer service/member services within the healthcare sector.
- Experience working with at-risk populations.
Skills
- Strong customer service knowledge, including needs assessment and quality standards.
- Ability to work well with peers, supervisors, and customers as a team member.
- Proactive, self-directed problem-solving and system planning abilities.
- Ability to manage multiple tasks and respond promptly to customer needs.
- Strong written and oral communication skills.
- Cultural competency and sensitivity toward the Medicaid population, including members with limited English proficiency.
- Awareness of member barriers related to transportation, childcare, healthcare knowledge, and trust in the system.
- Patience and empathy when interacting with members and internal/external customers.
Travel Requirement
This is a remote position requiring 50-75% local travel (reimbursable), with regular business hours Monday through Friday and occasional evening or weekend availability for community events.
